Donor information is automatically sent to the Donor Shield Insurance company and no further action is required. The default beneficiary is “the estate of donor.”

If I am NOT interested in either wage or travel reimbursement but I AM interested in having donation life insurance, donation disability insurance, coverage for uncovered complications and legal representation, do I need to provide my ACH information?

No, you do not need to provide ACH information or any additional documentation if you are not requesting wage or travel reimbursement. You are automatically enrolled in the other protections.
